| Health insurance is required of all UTHealth students. The Board of Regents has authorized the assessment of a health insurance fee for each semester to each student who cannot provide evidence of continuing coverage under another approved plan. The waiver form may be obtained from the Registrar's Office, Auxiliary Enterprises, or by clicking on this link: Health Insurance Waiver form. You can obtain details on the insurance plan in the Auxiliary Enterprises office. Their phone number is 713-500-8400. PLEASE NOTE: The form must be submitted no later than the 12th class day of the Spring and Fall term, and the 4th class day of the Summer term. If you do not take action by the deadline, you MUST pay the insurance assessed to you. |
