| Student Service Fee: includes access to (1) the UT Recreation Facility on Knight Road, (2) UT Health Services in UPB 510, (3) UT Shuttle Buses, (4) Student Government, and (5) UT Student Counseling Services in UCT 1600. The Student Health Service includes without charge sickness visits, physicals, gynecologic exams, treatment for needle sticks, and immunizations (excluding hepatitis B). The Student Health Services does NOT include free xrays, medical procedures, prescriptions or laboratory work. Student Health Service is not to be confused with medical health insurance. |
| Health insurance is required of all Health Science Center students. The Board of Regents has authorized the assessment of a health insurance fee for each calendar school year to each student who cannot provide evidence of continuing coverage under another approved plan. The waiver form may be obtained from the Registrar's Office, Auxiliary Enterprises, or by clicking on this link: Health Insurance Waiver form. You can obtain details on the insurance plan in the Auxiliary Enterprises office. Their phone number is 713-500-8400. PLEASE NOTE: The form must be submitted no latter than the 12th class day of the Dental School Year. If you do not take action by the deadline, you MUST pay the insurance assessed to you. |
