Academic History - Previous Education | Official Transcripts |
All admitted applicants to the Medical School must submit an official transcript from each previously attended institution of higher (i.e., post-secondary) education to both the TMDSAS and the admitting campus. Transcripts are due in the Registrar's Office by September 1 of the student's entering year.
The transcript should show a record of all academic course work. It must be submitted by the institution directly to the Office of the Registrar electronically or by mail at the following address:

| The first three years of Medical School are pre-determined curricula. Fourth year medical students pre-select their classes during the third year. Therefore, registration and enrollment to begin a Medical School Year does not involve the selection of classes. |

To be registered for a new year you must do the following:
- Clear any holds which exist at the beginning of the registration period. Check UTLINK to determine if holds have been placed on your record.
- Update your mailing address information via UTLINK.
- Use UTLINK to select payment by the Installment Plan if you wish to make four payments during the academic year, or call the Bursar's Office at 713-500-3088.
- UT System requires all Health Science Center students have health insurance coverage. Students will be automatically assessed the UT insurance plan but may have this fee waived by Auxiliary Enterprises if proof of comparable coverage is presented by the 12th class day.
- Bills will not be mailed. Students should view bills on UTLINK. Fees should be paid by the due date printed on the bill.

Immunizations - Year 1 students |
| The Texas Department of Health has issued immunization requirements for students in health-related programs. All students must have completed immunizations prior to registration. STUDENTS WILL NOT BE ALLOWED TO REGISTER WITHOUT VERIFIED IMMUNIZATION DOCUMENTATION. Records must be submitted to Health Services prior to registration. Health Services is located at UPB 510, 6410 Fannin, Houston TX 77030 . Telephone (713) 500-5171, fax (713) 500-0605.

The student is responsible for providing billing information to the Registrar's Office prior to the 12th class day for the current term. If the billing information is not in the Registrar's Office by these dates, students must pay tuition & fees to avoid cancellation. | A student who fails to make full payment of tuition and fees, including any incidental fees, by the due date, may be prohibited from registering for classes until full payment is made. A student who fails to make payment prior to the end of the semester may be denied credit for the work done that semester. |

| Students may pay Medical tuition and fees in full or by installments. Students may use UTLINK to select the installment plan by clicking on the Financial tab, and selecting Other from the menu. Students may also go in person to the Bursar's Office at UCT 2240. |
Special Note: Dropping classes or withdrawing from the University does not relieve a student of the responsibility for unpaid financial obligations. Students enrolled in an installment payment plan will be required to continue making payments until the non-refundable portion of their account is paid in full. Refunds or credits are based on the percentage of tuition and fees charged, not on the percentage of tuition and fees paid.
Special Note: Certain fees (health insurance, liability insurance, installment use fee, etc.) must be paid in full at the initial payment. |

| Students receiving financial aid for the first time must complete an entrance interview before their first disbursement of funds for the year. Please call the Financial Aid Office, 713-500-3860 or access the Student Financial Aid website to complete a financial aid entrance interview. |
